1. Sponsor

The sweepstakes (“Sweepstakes”) is sponsored by Full Circle Research Company, LLC, 11701A Falls Road Potomac, MD 20854, U.S. (“Sponsor”).

2. Eligibility

The Sweepstakes is open to Survey Roundtable panel members who comply with the following conditions and must be: 

(i) registered with the Survey Roundtable panel from June 1, 2013, until December 31, 2025; 

(ii) legal residents of the United States who are18 years of age or older, or of the legal age required to agree to and enter into the Sweepstakes; and 

(iii) active in the Survey Roundtable panel by completing at least one survey during the sweepstakes period (“Sweepstakes Period”) in which they want to enter, defined in table below.

Employees, officers, directors, agents and representatives of Sponsor, its affiliates, subsidiaries, subcontractors and their immediate family members (spouse, parents, children, siblings) and household members are not eligible. The Sweepstakes is subject to all applicable Federal, state and local laws and regulations and is void where prohibited.

3. Agreement to the Sweepstakes Rules

Participation in the Sweepstakes constitutes entrant’s full and unconditional agreement to and acceptance of these Sweepstakes Rules (“Rules”) and the decisions of the Sponsor, which are final and binding.

4. Sweepstakes Periods

The Sweepstakes begins (“Start Time and Date”) and closes (“Close Time and Date”) according to the table above. 

Entries submitted before or after each Sweepstakes Period will not be eligible for that Sweepstakes Period (for example, surveys completed in October are not counted as entries in November).

5. How to Enter

Eligible participants who complete a survey during a Sweepstakes Period are automatically entered in that specific Sweepstakes Period. To enter without completing a survey, please contact us at SupportUS@surveyroundtable.com from your Survey Roundtable membership email address on or before the Sweepstakes Period close date and time. In the email, include your full first name and last name initial, as well as your city and state of residence. For specific close dates and times, see the table above.

Only one email entry per person is permitted. All email entries must have the valid email address used for the Survey Roundtable panel. To ensure receipt of future email communication, please add SupportUS@surveyroundtable.com to your safe senders list.

6. Prizes and Winner Selection

There are three prizes available per Sweeptstakes Period noted in the chart above, assigned in a random draw. First-place winners receive a $200.00 Amazon.com Gift Card, second-place winners receive $100.00 Amazon.com Gift Card and third-place winners receive a $50.00 Amazon.com Gift Card.

Prizes are non-transferable and non-assignable. No substitutions of prizes are permitted except by Sponsor. Prizes are not redeemable for cash. All prizes will be awarded. The odds of winning are dependent upon the number of eligible entries received. Prizes will be transferred to the winners via Survey Roundtable email account within 5-10 working days after receiving a reply to the notification.

Three entries will be selected via random draw from all eligible entries received during a Sweepstakes Period. The Sweepstakes draw will be conducted on the draw dates referenced in the table above. In order to be declared a winner, the winning entries must be verified by Sponsor. Sponsor reserves the right to require proof of age and jurisdiction of residence prior to awarding any prize.

8. Verification of Potential Winners

Potential winners are subject to verification of eligibility and compliance with these Rules. Sponsor’s decisions are final. Winners may be required to sign and return: an affidavit of eligibility, liability/publicity release form and tax forms (e.g., IRS Form W-9) as a condition of receiving the prize. Selected winners will be notified by email, within 48 business hours of the draw date. The selected winners will be required to acknowledge, via web form, eligibility for the prize. If no response is received by Sponsor within 10 business days of notification, the prize will be forfeited and may be awarded to an alternate winner in a random draw selected from the remaining eligible entries.

9. Publicity

Except where prohibited, participation in the Sweepstakes constitutes winner’s consent for Sponsor to use winner’s name, likeness and entry for promotional purposes in any media, without further payment or consideration.

10. General Conditions

Sponsor is not responsible for any costs associated with claiming any prize. Winners are solely responsible for the reporting and payment of all taxes, if any, that may result in claiming a prize in this Sweepstakes. 

Entries in any way tampered with, forged, incomplete, illegible or mechanically reproduced, or submitted by or through fraudulent means, may be disqualified, at the sole discretion of Sponsor. This includes any entry that does not comply with these Rules in any respect. 

Decisions of Sponsor are final on all matters of fact, interpretation, eligibility, procedure and fulfillment in respect to this Sweepstakes. Sponsor reserves the right at any time to withdraw this Sweepstakes or to modify, amend or supplement these Rules, in its sole discretion, subject to applicable law. 

By participating in this Sweepstakes, the winners consent to the use by Sponsor of their name, town or city, age and photograph for any lawful purpose, without further permission or compensation, except where prohibited by law. The winners agree to abide by and be bound by these Rules.   

All applicable Federal, state and local laws apply. Sponsor also reserves the right to disqualify any individual who tampers with the entry process or otherwise violates these Rules.

11. Release, Limitations of Liability and Disclaimer

Sponsor, its subsidiaries and affiliated companies and each of their respective directors, officers, employees, agents, representatives, advertising, promotion and fulfillment agencies, and legal advisors are not responsible for and shall not be liable for: (i) telephone, electronic, hardware or software program, network, Internet or computer malfunctions, failures or difficulties of any kind; (ii) failed, incomplete, garbled or delayed computer transmissions; (iii) any condition caused by events beyond the control of Sponsor that may cause this Sweepstakes to be disrupted or corrupted; (iv) any injuries, losses or damages of any kind arising in connection with or as a result of the prize, or acceptance, possession or use of the prize, or from participation in this Sweepstakes; or (v) any printing or typographical errors in any materials associated with this Sweepstakes.  Sponsor reserves the right, in its sole discretion, to suspend or cancel this Sweepstakes at any time if a computer virus, bug, tampering, unauthorized intervention or other technical problem of any sort corrupts the administration, security or proper play of this Sweepstakes. Sponsor also reserves the right to disqualify, in their sole discretion, any person tampering with the entry process, the operation of the Sponsor’s web sites or who is otherwise in violation of these Rules. Any attempt to damage SurveyRoundtable.com or undermine the legitimate operation of the Sweepstakes is a violation of criminal and civil laws. Should such an attempt be made, Sponsor reserves the right to prosecute violators to the fullest extent allowable by law.

By participating, entrants agree to release and hold harmless Sponsor, its subsidiaries and affiliated companies and each of their respective directors, officers, employees, agents, representatives, advertising, promotion and fulfillment agencies, and legal advisors from and against any claim or cause of action arising out of participation in the Sweepstakes or receipt or use of any prize. Sponsor is not responsible for lost, late, incomplete, inaccurate or misdirected entries, or for technical errors of any kind.

12. Disputes

Except where prohibited, entrants agree that any disputes arising under or relating to the Sweepstakes shall be governed by, and construed in accordance with, the laws of the State of Maryland, U.S., without regard to its conflict of law principles. All disputes shall be resolved individually, without resort to class action, and exclusively by the appropriate court located in Montgomery County, Maryland, U.S.
